We are seeking a dedicated and experienced Family Nurse Practitioner to join our healthcare team. The ideal candidate will provide comprehensive primary care services to patients of all ages, focusing on family-centered care and health promotion.

Responsibilities:

• Conduct comprehensive health assessments and screenings for individuals of all ages, taking into account physical, emotional, and environmental factors that may impact health outcomes.
• Develop individualized care plans that address the holistic needs of patients, incorporating evidence-based interventions, lifestyle modifications, and complementary therapies.
• Provide primary care services, including diagnosis, treatment, and management of acute and chronic health conditions such as diabetes, HTN, chronic pain, and asthma , with an emphasis on preventive care and health promotion.
• Order and interpret diagnostic tests such as lab work and medical imaging
• Prescribe medications and treatments
• Collaborate with patients and their families to set health goals and empower them to actively participate in their own care through education, coaching, and support.
• Offer holistic interventions and modalities to support healing and symptom management, such as nutrition counseling, stress reduction techniques, and integrative therapies.
• Collaborate with interdisciplinary team members to coordinate care and address the diverse needs of patients.
• Maintain accurate and thorough documentation of patient encounters, treatment plans, and outcomes in electronic health records (EHR) systems.

Qualifications:

• Master's or Doctoral degree in Nursing with specialization as a Family Nurse Practitioner.
• Current state licensure and certification as a Nurse Practitioner (FNP) with prescriptive authority.
• New graduates are wellcomed
• Focus on holistic healthcare preferred.
• Strong clinical assessment and diagnostic skills, with the ability to provide evidence-based care and promote health and wellness.
• Knowledge of integrative and complementary therapies, with a willingness to incorporate these modalities into patient care when appropriate.
• Excellent communication and interpersonal skills, with the ability to build rapport and establish therapeutic relationships with patients and families.
• Commitment to cultural competence, diversity, and inclusivity in healthcare delivery.

Contract Payment

• Competitive fee-for-service reimbursement rates commensurate with experience and productivity.
